About Job

Xilin Association seeks a Social Worker in Chicago Metropolitan Area. This position will provide psychosocial support to individuals, families, and groups, assisting them in coping with chronic, acute, or terminal illnesses. Responsibilities include home

visit, counseling, client and caregiver education, case management, and advocacy. The role involves developing and modifying care plans, collaborating with multidisciplinary teams,

connecting clients with community resources, and addressing barriers to healthcare. The position also includes crisis intervention, elderly abuse investigations, and facilitating support

groups, all aimed at promoting health and improving community well-being.

The essential job duties of this position include:

Ø Assessments and Care Planning:

· Conduct comprehensive assessments of clients’ mental, physical, and social needs;

· Develop and implement individualized care plans, identifying goals with clients and strategies to meet them;

· Monitor client progress and adjust care plans as needs change.

Ø Counseling and Crisis Intervention:

· Provide counseling services to individuals, families, and groups, addressing challenges such as chronic illness, grief, and life transitions;

· Respond to crises with immediate interventions to stabilize and support clients.

Ø Resource Advocacy and Referrals:

· Assist clients in accessing community resources, including healthcare, housing, and financial aid;

· Advocate for clients by removing barriers to services and ensuring proper follow-through.

Ø Collaboration and Community Engagement:

· Work with other professionals, such as healthcare providers and community organizations, to coordinate care;

· Participate in multidisciplinary team meetings and case conferences to ensure seamless service delivery;

· Stay informed about community resources and relevant legislation to provide accurate guidance.

Ø Training, Education, and Supervision (Has MSW and LSW):

· Facilitate support groups and workshops for clients, families, and caregivers;

· Mentor BSW interns who will finish their field placement at Xilin and to foster their professional development.

Ø Administrative and Reporting Duties:

· Maintain accurate and timely documentation of client interactions and care plans;

· Track compliance with care standards and organizational protocols;

· Participate in program evaluations to ensure service quality and effectiveness.

Job requirements:

· Master’s degree in Social Work from an accredited university.

· Licensed Social Worker (LSW) in Illinois.

· At least 6 months of experience in counseling, crisis intervention and case management.

· Knowledge of Loss and Grief theory and trauma-informed care.

· Skilled in crisis intervention techniques, survivor-centered counseling approach, and crisis management techniques

· Skilled in assessments, individualized care planning, and advocacy for community resources.

· Exceptional communication and interpersonal skills, with bilingual proficiency in English and Mandarin Chinese.

· Strong organizational, time management, and problem-solving abilities.

· Ability to work independently and collaboratively in team settings.
