- Assess referrals for appropriateness for home health, hospice, infusion, and related services, including medical, physical, social, and emotional needs, and determine equipment and supply requirements.
- Coordinate discharge planning with hospitals, clinics, physicians, and other community referral sources to ensure smooth transitions of care.
- Provide education and consultation to hospital staff, patients, families, and physicians regarding available home health, hospice, and infusion services.
- Serve as liaison between patients, families, physicians, payors, and referral sources, ensuring accurate documentation and physician orders are provided to intake.
- Collaborate with attending physicians, Hospice Medical Directors, hospitalists, and other staff as needed to support patient care and service appropriateness.
- Facilitate admissions for Home Health, Hospice, and Infusion Pharmacy, ensuring required consents and documentation are obtained.
- Support the implementation and growth of the General Inpatient (GIP) Hospice Program, including discharges to home, skilled nursing facilities, or assisted living facilities as appropriate.
- Participate in meetings to promote home health, infusion, hospice referrals, and GIP services.
- Provide staff and physician education on end-of-life care, hospice, and GIP services.
- Evaluate and document referrals in the Providence electronic medical system; track referral outcomes across nursing units and agencies.
- Maintain and expand referral base relationships by serving as a resource for education and information.
- Participate in strategic initiatives to increase referrals and support business growth within assigned accounts.

- Required - Bachelor’s Degree in Business, Healthcare Administration, Marketing, or a clinical discipline, or equivalent education/experience.
- Preferred - Coursework or training with a focus on healthcare.
- Preferred - Prior experience in sales within home health, hospice, or infusion services; direct sales to physicians, hospitals, or discharge planners; and experience with customer relationship and territory management software.
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
- Other Skills and Abilities: Strong verbal and written communication skills in English, with the ability to communicate clearly, sensitively, and professionally with patients, families, caregivers, co-workers, and the public. Demonstrated ability to build and maintain collaborative, constructive interpersonal relationships and to practice effective teamwork. Strong problem-solving, negotiation, and public relations skills with the ability to support families in crisis. Ability to prioritize multiple tasks, follow appropriate communication channels, and adapt to dynamic environments while remaining calm under pressure. Competence in customer service communication across phone, email, and in-person interactions.
