Position Summary:
Responsibilities include but are not limited to taking a referral that is submitted and, abiding by all regulations, to get the referral to an active patient.
Qualifications Requirements:
To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Home health care knowledge a plus. In addition, maintaining a professional and positive demeanor at all times are essential.
Language Skills:
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to effectively present information in one-on-one and small group situations to clients and other employees of the organization.
Other Skills and Abilities:
Basic computer skills required. The ability to utilize basic computer software integral to agency operations after initial training period and multi-tasking capability is a must.

- Follow through with the referral process, abiding by all regulations, to get the referral to an active patient.
- Understand the MLTC enrollment process and how Medicaid is obtained
- Assess the referral throughout the enrollment process to ensure a smooth transition
- Possess professional phone skills when dealing with potential client and or insurance company
- Additional duties as assigned by the Director
- Assist with agency job functions as needed due to absence and/or agency overflow
