About Job

Provides housing clients with assistance in maintaining housing to avoid homelessness. Also provides information and referrals to clients and/or caregiver(s) regarding housing options, rights and programs within the Houston/Harris County.

Duties and Responsibilities:

  • Seeks to assist clients to maintain long-term housing.
  • Collaborates with the Director of Housing Services and fiscal staff to assure that all required income and financial assistance documentation is secured, and assistance is provided in a timely manner.
  • Provide outreach to landlords and clients to market the Permanent Supportive Housing program.
  • Conduct face-to-face housing presentations.
  • Contact prospective landlords to schedule showings of rental units.
  • Schedule housing interview for consumers.
  • Assist with the rental application process; and help clients make informed decisions.
  • Accurate and timely data management and reporting.
  • Provides services through face-to-face and phone consultations; conducts home visits at least once a month or as needed.
  • Maintain current information about consumers’ benefits (basic income and medical information) to ensure that the subsidy amount is accurate about consumer’s income.
  • Assists clients with other social resource navigation as needed
  • Works as part of the care team to provide community health worker and behavioral health resources as needed.
  • Participates in monthly care team meetings to provide updates and identified needs
  • Deliver services with safety, respect, and effectiveness
  • Other duties as assigned
  • Housing referral experience with local, state and/or federal programs
  • Experience working clients with HIV/AIDS preferred
  • Experience working in a Federally Qualified Health Center (FQHC)
  • Preferred: Bilingual (English/Spanish).
  • Certified Community Health Worker (CHW) or must obtain certification with 90 days of hire.
  • BA/BS preferred.
