Job Overview – Licensed Social Worker (MSW) – Healthcare:
Compensation: $43/hour
Location: Queens, NY
Schedule: Monday to Friday, 9:00 AM – 5:00 PM
Our healthcare client is hiring a Licensed Social Worker (MSW) in Queens, NY, to provide case management, advocate for participants, and coordinate care across hospital, outpatient, and community settings. You will conduct assessments, develop care plans, and connect patients and families with essential resources while collaborating with an interdisciplinary healthcare team to improve quality of life and support safe transitions of care.
Responsibilities as the Licensed Social Worker (MSW):
- Advocacy & Casework: Provide ongoing case management to help participants and their families understand, accept, and follow care recommendations.
- Care Coordination: Oversee hospitalizations and discharges, ensuring timely transitions and arranging follow-up services for safe community-based care.
- Community Resources: Educate participants and families on accessing health and welfare programs to promote safety, independence, and quality of life.
- Assessment & Planning: Complete psychosocial and mental status assessments using systems such as McKesson and Care Compass, and develop individualized care plans.
- Crisis Support: Deliver crisis intervention, counseling, and emotional support, including for terminally ill patients and families in need of bereavement services.
Qualifications for the Licensed Social Worker (MSW):
- Education: Master of Social Work (MSW) degree from an accredited institution required.
- Experience: Minimum of 1 year of social work experience in a healthcare setting required.
- Licensure: Current New York State license as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required.
- Technical Skills: Proficiency in assessment platforms such as McKesson and Care Compass.
- Industry Knowledge: Strong knowledge of healthcare systems, discharge planning, and community resources.
- Skills & Attributes: Excellent case management, communication, and problem-solving skills, with the ability to collaborate effectively in a multidisciplinary environment.
