PSS may work with the Behavioral Health Response Team (BHRT), as needed, to build rapport with individuals struggling with substance use and mental health concerns. In addition, PSS will collaborate closely with Cone Health to provide consistent engagement and follow-up to individuals who have experienced an accidental overdose.Key Responsibilities: Build trust and rapport with referred individualsConduct peer support, education, and outreach servicesFacilitate access to emergency shelter and housing optionsProvide information and referrals for basic needs, medical care, mental health, and substance use treatmentAssist with connecting individuals to County or State benefits (e.g., disability, food stamps, Medicaid)Provide crisis intervention when necessaryCollaborate with local agencies, community partners, and Greensboro Police Department officersParticipate in regular meetings and trainings to stay informed and enhance service deliveryCoordinate referrals and care plans with community agencies and resourcesAttend community and provider meetings as neededMaintain accurate and up-to-date program records and documentation in compliance with policies and proceduresThis role is a critical part of improving access to services and support for vulnerable individuals in our community.
