Job Title: Licensed Social Worker

Job Summary
We are seeking a Licensed Social Worker to join our healthcare team in Chicago. The Social Worker will provide direct mental health services within school-based telehealth clinics, working closely with students, families, and providers to support emotional well-being and restore patients to their highest level of social functioning. This role requires strong clinical assessment skills, crisis intervention experience, and the ability to work independently while collaborating with a multidisciplinary care team.
Key Responsibilities
Case Management
- Develop and implement social work treatment plans based on patient and family needs.
- Conduct therapy sessions and crisis interventions as required.
- Initiate and participate in case conferences.
- Create aftercare and discharge plans for patients.
Patient Assessment and Treatment
- Perform thorough social assessments and document findings accurately.
- Provide individual therapy, casework, and crisis counseling.
- Coordinate with external treatment providers and advocate for patients and families.
Documentation and Compliance
- Complete all clinical documentation in compliance with HIPAA, NASW standards, and Illinois Mental and Public Health Codes.
- Work with billing teams to resolve authorization and reimbursement issues.
- Maintain accurate case records and reports.
Collaboration and Teamwork
- Communicate effectively with physicians, nurses, and other team members.
- Provide education and orientation to new staff, students, and volunteers.
- Participate in safety programs, in-services, and risk management initiatives.
Patient & Family Support
- Explain care processes clearly to patients and families.
- Actively listen and provide appropriate counseling and resources.
- Advocate for patient rights and access to needed services.
Qualifications
Required
- Master’s degree in Social Work (from a CSWE-accredited program).
- Active Illinois Licensed Social Worker (LSW).
- At least one year of experience in a healthcare or clinical setting.
- Strong communication skills, verbal and written.
- Ability to work with adolescent patients in varying emotional states.
Preferred
- Experience in school-based or community health settings.
- Crisis intervention and emergency response training.
- Knowledge of public health systems and community resources.
