Job Summary

We are seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our Behavioral Health Services team in Chicago. This role focuses on providing psychosocial assessments, counseling, and support to patients and families facing illness, disability, or loss. The LCSW will work closely with healthcare providers and community organizations to ensure patients have access to resources that improve their well-being and support their recovery journey.

Key Responsibilities

• Conduct psychosocial assessments, develop care plans, and document interventions in a timely, accurate manner.
• Reassess patients as health status changes, updating treatment and discharge plans as needed.
• Provide individual, family, and group counseling sessions.
• Coordinate with community and government agencies to connect patients with appropriate resources.
• Offer consultation to healthcare teams on psychosocial management of patients and family dynamics.
• Educate patients and families on available services, coping strategies, and health-related issues.
• Maintain confidentiality and follow professional ethical standards at all times.
• Participate in team meetings, case conferences, and interdisciplinary collaboration.
• Support patients with crisis intervention and emergency counseling when needed.
• Carry out other duties as assigned.

Qualifications

Required:

• Master’s degree in Social Work (from an accredited program).
• Current Illinois state license as a Clinical Social Worker (LCSW).
• Strong understanding of psychosocial issues impacting patients and families.
• Excellent communication, assessment, and counseling skills.
• Ability to cope with stress and maintain accuracy in a fast-paced environment.

Preferred:

• Experience working in hospital, behavioral health, or community health settings.
• Familiarity with local community resources and support systems.
• Training in crisis intervention techniques.
