Job Description:
We're looking for a dedicated and skilled Care Coordinator, Long-Term Care (Bilingual Spanish/English) to join our team. In this role, you will provide comprehensive healthcare management services to members enrolled in our Medicaid Long-Term Care/Comprehensive Program. Your primary goal will be to facilitate care coordination, manage costs, and improve healthcare outcomes by building strong relationships with our members through proactive engagement.
Responsibilities:
- Conduct comprehensive case management assessments, both over the phone and in person.
- Coordinate care for members of the Medicaid Long-Term Care/Comprehensive Program.
- Develop, implement, and monitor personalized care plans.
- Lead multidisciplinary reviews to ensure the best possible outcomes for members.
- Review and escalate quality-of-care issues as needed.
- Use negotiation and motivational interviewing skills to secure appropriate services and benefits for members.
- Educate and coach members and their caregivers, empowering them to make independent health decisions.
- Maintain strict compliance with all regulatory, accreditation, and company guidelines.
Required Qualifications:
- Bachelor’s degree is required, with a preference for Social Work or a related field. (Please note: a nursing degree is not a substitute for this requirement.)
- 1+ years of case management experience is a must.
- Must be bilingual in Spanish and English.
- Proficient in Microsoft Office, including Excel, Word, and Outlook.
- Strong interpersonal, communication, and motivational interviewing skills.
Preferred Qualifications:
- Case Management Certification.
- Experience in long-term care.
- Experience in managed care and discharge planning.
