Position Summary
The Community Liaison and Outreach Specialist will represent Holding Hands Counseling Services Inc.—an AHCA-licensed Community Mental Health Center—in Brevard and Central Florida. This role is responsible for building strong community partnerships, increasing awareness of our counseling and behavioral health programs, and ensuring that individuals and families are connected to high-quality, culturally relevant mental health services.
Key Responsibilities-Outreach & Engagement
- Develop and implement outreach strategies to increase referrals from primary care physicians, hospitals, schools, and community agencies.
- Represent Holding Hands at health fairs, provider meetings, and community events.
- Deliver presentations to educate the public and professionals about our counseling and case management services.
Relationship Building
- Build and maintain partnerships with healthcare providers, social service agencies, churches, and nonprofits.
- Serve as the primary contact for referral sources and ensure excellent communication and follow-up.
- Thank and maintain relationships with offices that receive our staff for outreach visits
Program Support
- Support intake and referral coordination by linking patients directly to our licensed providers and services.
- Track outreach activities, referrals received, and outcomes to demonstrate program growth and community impact.
- Work closely with the clinical and administrative team to align outreach with program capacity and service goals.
Advocacy & Communication
- Advocate for community mental health needs and share insights with leadership for service improvement.
- Communicate program updates, referral procedures, and success stories to stakeholders.
Provide ongoing education about the role of community mental health centers.
Qualifications
- Education: High School Diploma or GED required; Associate’s degree in Human Services, Social Work, Psychology, Public Health, or a related field preferred.
- Experience:
- 1–2 years of experience in outreach, customer service, healthcare, or community engagement.
- Familiarity with mental health, healthcare, or social service systems is helpful but not required.
Skills:
- Excellent communication and networking skills.
- Bilingual (English/Spanish) strongly preferred.
- Ability to work independently and manage multiple tasks.
- Knowledge of Brevard/Orlando community resources is a plus.
- Basic computer skills (Microsoft Office, email, outreach tracking).
Work Environment
- Based in Melbourne, FL, with outreach responsibilities across Brevard.

Performance Indicators
- Growth in patient referrals from PCPs and community partners.
- Expansion of provider network relationships (Florida Blue, Medicare, and other insurers).
- Documented community presentations and outreach events.
