Social Worker

POSITION SUMMARY – Social Worker

POSITION DUTIES –

• Perform biopsychosocial evaluations in accordance with departmental policies and standards.
• Document notes related to ongoing work in a timely and comprehensive manner.
• Collaborates with MD in planning and participating in family meetings.
• Provides support and education to patients and families related to illness, hospitalization, and post discharge options and follow up.
• Assists in facilitating the continuum of care and accessing community services for patients and families as needed.
• Completes insurance reviews as indicated.
• Utilizes supervision by bringing appropriate materials for discussion and following up on recommendations.
• Participates in daily rounds and consults with other clinical staff to facilitate enhanced patient care.
  
  

MINIMUM REQUIRED QUALIFICATIONS – LCSW licensure with plan for LICSW (MA licensed); MSW degree from accredited school; BLS; 2 years of experience in health care, with special interest and understanding of mental health diagnosis, substance issues, and interpersonal/domestic abuse.

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8am-6:30pm

START DATE – ASAP
