Primary Responsibilities Include:

  • In the role, the Intake Coordinator works in a fast-paced environment responding to clinical professionals, prospective clients and their family members seeking treatment.
  • Determines whether patient meets admission criteria for inpatient treatment care, and arranges such admission if appropriate.
  • Day-to-day, you would handle primarily inbound calls, trying to connect as many people as possible to the correct professional team member.
  • Receives referrals of individuals who are of an at-risk population from case managers, staff nurses, team members, physicians, and external referral sources
  • Call quality & performance are carefully monitored and coached
  • Other duties as assigned

Requirements:

  • Masters degree in social work or related field required
  • Licensed clinician in the state of IL required
  • Prior experience working with Electronic Health Record systems (EHRs)
  • Must have strong computer proficiency with knowledge of Microsoft Office, Internet, and Email
  • Must be fully vaccinated against COVID-19 and have all boosters (if eligible)
