Primary Responsibilities Include:

In the role, the Intake Coordinator works in a fast-paced environment responding to clinical professionals, prospective clients and their family members seeking treatment.
Determines whether patient meets admission criteria for inpatient treatment care, and arranges such admission if appropriate.
Day-to-day, you would handle primarily inbound calls, trying to connect as many people as possible to the correct professional team member.
Receives referrals of individuals who are of an at-risk population from case managers, staff nurses, team members, physicians, and external referral sources
Call quality & performance are carefully monitored and coached
Other duties as assigned

Requirements:

Masters degree in social work or related field required
Licensed clinician in the state of IL required
Prior experience working with Electronic Health Record systems (EHRs)
Must have strong computer proficiency with knowledge of Microsoft Office, Internet, and Email
Must be fully vaccinated against COVID-19 and have all boosters (if eligible)
