Role: Social Work Manager (MSW)
Location: San Jose, CA
Type: Full-time, Permanent
Work Arrangement: Onsite/field — center-based leadership with occasional home/community visits

Key Responsibilities

• Lead, hire, train, supervise, and coach Social Work staff (incl. Behavioral Health Specialists/Social Work Assistants)
• Oversee assessments, IDT participation, and care-plan execution
• Ensure compliance in documentation, assessments, care-plan completion/review, participant rights & grievance processes
• Co-lead programming (support groups, counseling, family education) with Behavioral Health; partner with community professionals
• Participate in end-of-life care coordination and support
• Conduct home visits as necessary

Requirements

• Master’s degree in Social Work (MSW)
• Social work licensure preferred (e.g., LCSW)
• 2+ years prior social work supervision/leadership strongly preferred
• 1+ year experience with frail/elderly population
• Excellent clinical, organizational, and communication skills across participants/families/IDT
• Valid driver’s license and current vehicle insurance

Preferred Skills

• LCSW; PACE/geriatric/home health/hospice experience; program development exposure

Job Type: Full-time

Pay: $105,000.00 - $140,000.00 per year
