As a designated Center of Excellence (COE) for spine surgery, our Spine Patient Navigator serves as the main point of contact for non-clinical responsibilities for patients referred to our practice from initial referral, evaluation, and pre and post-surgery for spine patients and their caregiver participating in our third-party contracted value-based care program.
Ideal candidates will have experience in a busy medical practice or hospital in a similar or other patient-focused role. You must be well-organized, adaptable, and possess attention to detail, advanced excel skills, and the ability to juggle competing priorities. You must possess excellent communication skills, critical thinking skills, and you must be a self-starter who can work independently. You must be able to easily travel between home, office, and other medical facilities in the Charlotte area in order to meet the requirements of this position. You must be available 24/7 via a pager or phone for urgent non-clinical situations such as travel delays or other issues that require immediate action outside normal business hours.

- Initiates contact with the patient and healthcare facilities to obtain medical records and imaging studies for further evaluation by the clinical case manager and/or surgeon to determine qualification for the program.
- Coordinates travel arrangements and lodging for patients and their caregiver in line with the program parameters.
- Meets qualified patients and their caregivers in the designated medical facility and escorts them throughout the day to appointments, labs, radiology, testing, or other pre-op appointments.
- Provides education, answers questions, provides assistance, and regularly communicates with the patient’s caregiver before, during, and after the patient’s surgery. This may include participating in consults with the surgeon.
- Identifies and educates caregiver on area dining, transportation, and pharmaceutical options in alignment with program parameters.
- Navigates barriers or challenges patient may be experiencing at home or in the medical facility that may impact safety or compliance.
- Maintains regular contact with patient and caregiver post-surgery at facility or hotel and resolves issues and responds to requests.
- Maintains necessary records in Excel, the EMR, and other required applications for the value-based care program.
- Assists with the resolution of any related billing and/or charge issues.
- Participates in department meetings, calls, and huddles to discuss patient care/program direction.
- Complies with all our processes and HIPAA regulations.
- Applies our values in decision-making and interactions with all individuals.

- Other duties as assigned by changing needs, patient flow, or physician request.
