Job Overview
We are seeking a dedicated and compassionate Home Health Physical Therapy Care Manager to join our team. This role is focused on delivering high-quality physical therapy services to patients in their homes. The Care Manager will perform comprehensive assessments, create individualized care plans, educate patients and caregivers, and collaborate with an interdisciplinary team to ensure optimal patient outcomes. This position offers the opportunity to make a meaningful impact on patients’ independence, recovery, and overall well-being.
Key Responsibilities
- Manage and coordinate care for a caseload of home health patients requiring physical therapy as the primary service.
- Conduct thorough assessments of functional abilities, including mobility, daily living activities, and use of assistive devices.
- Develop individualized, measurable, and realistic care plans in collaboration with patients, families, and physicians.
- Provide therapy services, monitor progress, and adjust treatment plans as needed to support patient goals.
- Reassess patients regularly and document progress, effectiveness, and changes in condition.
- Educate patients, families, and caregivers on safe and effective care techniques.
- Collaborate with interdisciplinary teams to ensure comprehensive care and appropriate referrals.
- Maintain accurate clinical documentation in compliance with standards and regulations.
- Identify opportunities for quality improvement and support initiatives that enhance care delivery.
Requirements
- Licensed Physical Therapist (PT).
- Minimum of 1 year of relevant clinical physical therapy experience.
- Valid driver’s license and proof of current auto insurance.
- Current Basic Life Support (BLS) certification.
Preferred Qualifications
- Doctorate degree in Physical Therapy.
- At least 1 year of recent experience in a Medicare-certified home health agency as a care manager.
- Home Health Care-Manager Certification.
- Certification for Oasis Specialist-Clinical (COSC).
