Job Title: Part Time Assessment Intake Coordinator / Intake Coordinator

Position Overview: LINK of Hampton Roads is seeking an experienced Assessment Intake Coordinator (AIC) / Intake Coordinator to join our team. The AIC/Intake Coordinator will play a crucial role in evaluating programs and projects, conducting initial assessments of clients, and coordinating various aspects of client care. This position involves a combination of responsibilities, including program evaluation, data analysis, case assessments, and client support.

Key Responsibilities:

Program Evaluation: Identify the purpose of programs and projects, align them with the organization's vision and mission, and develop criteria for evaluation.

Data Collection and Analysis: Conduct housing and case record inspections, investigations, and data analysis to assess program effectiveness and make informed recommendations for improvement.

Client Assessment: Perform initial assessments of clients to determine mental health needs, establish care requirements, and gather essential information from federal and patient records.

Client Support: Address client inquiries related to services, provide information, and make referrals when necessary to ensure clients receive appropriate care and support.

Research and Feedback: Coordinate research, analysis, and studies of client feedback to gain insights into program performance and identify areas for enhancement.

Collaboration: Work closely with staff members to ensure effective coordination and communication within the organization.

Qualifications:

  • Bachelor's degree in a relevant field such as social work, psychology, or public health is preferred.
  • Experience in program evaluation, data analysis, or case management is advantageous.
  • Strong analytical and research skills.
  • Empathetic and effective communicator with clients and colleagues.
  • Knowledge of federal and patient records is beneficial.
  • Ability to handle a caseload of up to 100 veteran clients efficiently.
