We are currently looking to add Holistic Youth & Family Advisors (HYFA) to our team to provide Family Check-Up services.
The Family Check-Up Model is a unique, strength-based approach to improving family mental health, child behavior, and family relationships. The Family Check-Up Professional works with high and low risk youth and parents to provide parents with the tools and tailored support that they need to manage their children's behaviors and improve their parenting practices by enhancing the quality of all of their family relationships.
The Family Check-Up Provider will implement the evidence-based program, Family Check-Up, which is designed to address the most critical component of sustainability after the service ends, the parent. Purpose: To positively impact at-risk youth (ages 2-17) AND parents/legal guardians of at-risk youth that are faced with single to multiple barriers of success in life. Family Check-Up aims to improve parenting skills and family management practices, with the goals of improving a range of emotional, behavioral and academic outcomes.
Duties/Responsibilities:

  • Attend all service trainings, consultations, and supervisions to become qualified to provide the delivery of assigned treatment modalities.
  • Provide home and community-based services to identified at-risk families.
  • Learn and apply the Family Check Up Program, an evidence-based parenting intervention that incorporates motivational interviewing and the 5PHE engagement approach, family systems, and parent skills training
  • Link program participants to personalized follow up services
  • Assist and participate in special company programs and initiatives.
  • Successfully uphold and represent the Life Push, LLC brand of excellence and person-centered focus
  • Act as a resource and guide to youth and/or parents and legal guardians of at-risk youth facing single to multiple barriers of success in life.
  • Work closely with agency staff, case managers, and other case partners through phone contact, in-person conferences, and group meetings.
  • Serve as an example/role model upholding the Life Push brand and expectation of excellence at ALL TIMES.
  • Accept and relate to families who may not always share the HYFA's personal lifestyle and values.
  • Support the family while maintaining the proper respect for the family's right to self-determination and independence.
  • Attend required supervisions, company meetings and participate in both internal and external presentations
Requirements/Qualifications:
  • Must be able and willing to work flexible hours - (afternoon, evenings, and some weekends)
  • Must be at least 21 years of age
  • Must have a COMPLETED 4-Year degree in a human services or counseling related field REQUIRED
  • Must be willing to submit to and pass multiple thorough background checks
  • Must have a valid driver's license with the ability to provide a copy of your current driving record. (Driving record must be in good standing)
  • Must have reliable transportation with valid insurance
  • Must be willing to travel across listed counties to provide services
  • Open to cultural differences and willingness to go through company orientation and training before being paired with a client
  • Must have and demonstrate good listening skills, patience and a sense of awareness
  • Must adhere to deadlines and expectations for client safety.
  • Prefer at least 2 years of experience in counseling or mental health field &/or parent training programs working with families and/or parents/caregivers
