Overview
THP Case Manager & SUD Counselor provides integrated services for youth and young adults ages 16–25 enrolled in both the Transitional Housing Program (THP) and the Bridge to Wellness Program. This position combines case management and substance use counseling to support youth in achieving housing stability, independent living skills, and recovery goals. Many participants are foster youth, justice-involved, or experiencing housing instability. Services are field-based, trauma-informed, and culturally responsive, using the Seven Challenges® Model alongside individualized service planning and support.
Responsibilities
THP Case Management
- Conduct weekly home visits and one-on-one interactions with THP participants.
- Develop and update ANSP (Appraisal, Needs, & Services Plan) quarterly.
- Coordinate medical, dental, mental health, educational, and vocational services.
- Provide crisis intervention support and develop safety plans as needed.
- Submit timely and accurate case notes, SIRs (Special Incident Reports), and monthly money requests.
- Oversee emancipation funds and support youth transitioning out of THP.
- Communicate professionally with service providers, supervisors, and stakeholders.
- Participate in after-hours on-call rotation.
Substance Use Disorder Counseling (Bridge to Wellness)
- Provide individual and group counseling using the Seven Challenges® model.
- Develop treatment plans, monitor progress, and complete Medi-Cal compliant documentation.
- Deliver services in schools, homes, THP sites, and community settings.
- Engage families/caregivers when appropriate to support treatment outcomes.
- Participate in case conferences, reflective supervision, and interdisciplinary team meetings.
- Maintain fidelity to the Seven Challenges® model through training and supervision.
Cross-Program Collaboration
- Collaborate with partners (probation, foster care, housing, MHRS, ECM, schools) to ensure continuity of care.
- Seek guidance and accept coaching from supervisors and team members.
- Support data collection and reporting for grant compliance and program evaluation.
- Uphold confidentiality, mandated reporting laws, and professional boundaries at all times.
Knowledge, Skills, and Abilities
- Strong ability to build rapport with at-risk youth and young adults.
- Skilled in crisis intervention, harm reduction, and trauma-informed care.
- Effective written and verbal communication, including professional documentation.
- Problem-solving skills with ability to manage multiple priorities.
- Flexibility to work independently in a field-based setting.
Education and Experience
Required:
- Bachelor’s degree in social work, psychology, or related field OR equivalent lived/professional experience.
- Experience in case management and/or substance use counseling with youth or young adults.
- Willingness to complete Seven Challenges® training and maintain fidelity standards.
Preferred:
- Certification as a Substance Use Disorder Counselor in California (CADTP, CAADE, or CCAPP).
- Two years of case management experience.
- Bilingual (Spanish, Hmong, or other locally spoken languages).
- Experience with Medi-Cal documentation and grant-funded youth programs.

Job Type: Full-time
Pay: $27.00 - $33.00 per hour
