The Behavioral Health Care Coordinator gathers information for individuals establishing care within the recovery program, using standard protocols and assessment tools. The Care Coordinator is responsible for supporting and coordinating the needs of program participants, including internal referrals and connection to external resources. The Coordinator engages individuals in their identified goals and connects them to integrated, patient-centered services.

Salary begins at $24.26+ per hour depending on experience.
Essential Functions of the Job: [Other duties may be assigned.]
  • Completes screenings and performs review of intake materials requiring the use of independent judgment to complete recovery program intakes.
  • Schedule appointments for potential behavioral health clients and ensure timely communication with clinical staff regarding appointments.
  • Assist BH Services Manager with coordinating program access and admission priorities.
  • Participate in briefings, and other program meetings.
  • Work collaboratively with team members to connect people to whole-person care, including clinical needs and social determinants of health.
  • Completes routine screening, brief intervention and referral to treatment to promote service integration.
  • Promote timely access to appropriate care to reduce emergency room utilization and hospital readmissions.
  • Increase utilization of preventative care.
  • Connect people to resources and supports to engage in identified goals and treatment plans.
  • Mutually establish acceptable boundaries with participants and care team members.
  • Assist BH Services Manager by serving as an informational resource for participants, care team, family/caregiver(s), and community resources.
  • Facilitate access to appropriate medical and specialty providers.
  • Increase continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals.
  • Educate participants and family/caregiver(s) about relevant recovery resources.
  • Facilitate and attend meetings between participant, family/caregiver(s), care team, and community resources, as needed.
  • Assist with population health management initiatives.
  • Participate in Care Coordinator training courses/webinars and meetings.
  • Assist Integration team with quality improvement efforts by utilizing dashboards, tracking tools and providing quality reporting data as needed.
  • Supports accurate and timely documentation and audit efforts, using Electronic Health record system for documentation.
  • Understand and demonstrate knowledge of HIPAA and maintain strict confidentiality regarding protected health information.
  • Understands and upholds the organization’s stated Mission, Vision, Values and Goals.
  • Prioritize concurrent tasks and team requests to meet tight deadlines; flag obstacles and propose solutions in a timely manner.
Qualifications and Education Requirements
Bachelor degree in social science field (Social Work, Psychology, Sociology, etc.) required with at least 2 years of experience working in behavioral health or social services. (Masters in Social Work preferred).
Equivalent combination of education and experience may be considered. Physical Demands
