POSITION SUMMARY:

The Housing Clinician provides heavy, hands-on consult to the Living Well at Home Team and occasional in home therapy to clients in supportive housing. The Housing Clinician provides mental health and substance use evaluation, support and referrals during transitions of care between acute and community based settings. This role requires assertive outreach, advocacy, system navigation and service coordination to manage clients’ needs as they navigate the housing search process and transition into permanent housing.

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Manage clinical needs of a complex case load of clients with serious mental illness and/or substance use disorders

  • Address client behavioral health crises by providing crisis intervention, assessment and triage, de-escalation by utilizing health system and community based resources

  • Support client driven clinical goals by referring clients to ongoing mental health treatment, substance use treatment, and coordinating with other providers

  • Complete clinical documentation, assessments, and service plans in a timely manner

  • Work independently and as part of an interdisciplinary team

  • Meet with clients in the community, including hospital and home settings, or in the LWAH office Actively participate in all scheduled meetings, including team meetings, group supervision, and individual supervision

  • Using a trauma and harm-reduction informed lens, maintain appropriate boundaries with clients and staff Provide individual psychotherapy sessions to clients in the community as indicated Provide clinical consult to case management staff

JOB REQUIREMENTS

EDUCATION:

  • Master’s degree in social work or mental health counseling required

  • LCSW, LMHC, or LICSW required.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

  • Requires a Massachusetts license as a licensed mental health professional such as Licensed Certified Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC)

EXPERIENCE:

  • 2+ years working with patient populations impacted by serious mental illness and substance use disorder

  • Experience working in homeless services preferred

  • Experience working in an independent role preferred

  • Experience with community based work is preferred

  • Lived experience is valued

KNOWLEDGE, SKILLS & ABILITIES (KSA):

  • Ability to work with a diverse and highly impacted client population using a person-centered approach

  • Comprehensive knowledge of behavioral health and housing resources and systems, specifically in Greater Boston Area

  • Skilled at engaging difficult to engage patients—build rapport, trust and utilizing de-escalation skills

  • Ability to work in a fast paced environment, inclusive of organizational skills, time management skills, critical thinking, and problem solving abilities

  • Strong interpersonal skills and ability to communicate in a courteous, pleasant and professional manner with healthcare providers, patients and families, general public, staff members, external agencies, and other organizational leaders.

  • Ability to work independently and to make decisions based on established health system policies and procedures as well as the NASW or professional organization code of ethics

  • Proficient oral and written English communication skills.

  • Bi-lingual and/or bi-cultural candidates preferred

  • Intermediate Microsoft Office suite experience (i.e. MS Word, Excel, Access, Outlook)

  • Experience using an electronic health record preferred
