At Family Residences and Essential Enterprises, Inc. (FREE), we empower over 5,500 people with intellectual/developmental disabilities and mental health challenges to thrive in their communities. FREE provides a diverse array of supports and services including: housing; recovery services; transition to work; employment; day, community and family services; respite; crisis services; education and after school support; specialty health services; and advocacy.

We are seeking a Licensed Clinician for clinical oversight of people diagnosed with a variety of mental health and/or substance abuse and/or behavioral conditions. The Licensed Clinician provides crisis intervention, direct support, medication monitoring and oversight, assistance with daily living skills, and any other assistance that individuals need while participating in site-based services. You will assist in the development of the Initial Plans, Relapse Prevention Plans, and Support Plans. In addition, you will also run therapy groups on specific topics, provide individualized supportive counseling, and conduct assessments.

In addition, you will:

Perform assessments to evaluate a variety of mental health and/or substance abuse and/or behavioral conditions to children, adolescents and adults.
Develop individualized, prescriptive treatment plans to fit the needs of each patient using appropriate diagnostic and patient placement criteria.
Conduct individual, family, and group counseling, utilizing methods consistent with evidence-based practice, using a brief, focal model of care when appropriate.
Utilize outcome tools to determine patient progress.
Coordinate client care with primary care, other specialty providers, schools, and families.
Arrange for appropriate care within the FREE umbrella or through outside providers.
Ensure that each client's rights are protected and that they are provided with appropriate interventions based on ability to participate, cultural concerns, and diagnostic impression.
Participate in departmental activities such as clinical care conferences, integrated care committees, and risk meetings to ensure the highest quality of care.
Maintain progress notes and other documentation to ensure that the client's medical record is completed contemporaneously.

We require:

Master’s Degree in Human Services
Licensure as either an LMSW, LCSW, or LMHC
Valid NYS Driver’s License
Minimum of one (1) year of experience working with individuals with behavioral health diagnoses
Computer literacy and well-organized, with excellent communication skills
Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs, is essential
