About Job
- Assessing patient needs: conduct initial and ongoing assessments of patients’ health status to determine care requirements.
- Coordinating care plans: collaborate with doctors, nurses, social workers, and other healthcare professionals to develop and implement individualized care plans.
- Monitoring patient progress: track patients’ response to treatment and adjust care plans as needed.
- Facilitating communication: act as a liaison between patients, families, and the healthcare team to ensure clear communication and understanding of care goals.
- Discharge planning: prepare patients for discharge by arranging follow-up care, home health services, or referrals to other facilities.
- Teamwork: Support physicians and patients by managing appointments, coordinating care, monitoring patient goals, documenting health status, and facilitating communication to improve clinic services and reduce hospitalizations.
- Current and valid state license as an LPN.
- Current BLS (AHA) certificate upon hire and maintain current.
- Minimum one year of experience in an applicable setting preferred.
- IV certification upon hire.
