ResponsibilitiesEssential Duties and Responsibilities (includes, but are not limited to):

Assessing patient needs: conduct initial and ongoing assessments of patients’ health status to determine care requirements.
Coordinating care plans: collaborate with doctors, nurses, social workers, and other healthcare professionals to develop and implement individualized care plans.
Monitoring patient progress: track patients’ response to treatment and adjust care plans as needed.
Facilitating communication: act as a liaison between patients, families, and the healthcare team to ensure clear communication and understanding of care goals.
Discharge planning: prepare patients for discharge by arranging follow-up care, home health services, or referrals to other facilities.
Teamwork: Support physicians and patients by managing appointments, coordinating care, monitoring patient goals, documenting health status, and facilitating communication to improve clinic services and reduce hospitalizations.

QualificationsBefore we go any further, we do have some deal-breakers. You must have:

Current and valid state license as an LPN.
Current BLS (AHA) certificate upon hire and maintain current.
Minimum one year of experience in an applicable setting preferred.
IV certification upon hire.
