Fast-paced, rapidly growing primary care focused MSO seeks high energy, super organized, multi-tasking population health/value-based care manager to support our emerging physician support team.

Our company was founded on the value of the doctor-patient relationship, especially primary care. Our physicians provide extraordinary care to their adult patients through their participation in the Medicare Shared Savings Program. For the last 5 years we have had the highest quality score of all NYS MSSP ACOs!

We are seeking a manager to expand and develop our Physician Support Team, with responsibility for:

  • HCC documentation and coding, persistency and new diagnosis codes; you are comfortable with ICD10s and Excel
  • Quality Measures (HEDIS, ACO & Payor Specific) - you teaching and cheerleading to get the highest score possible
  • Cost of Care - you work with Analytics Team and the physician sites to ensure steps are taken to help these patients with catastrophic events or multiple chronic conditions
  • Proactive management of preventive care to ensure our patients all receive their full preventive care under Medicare and MA
  • We are seeking an entrepreneurial leader to grow and mentor the team

The position is based in Great Neck with periodic onsite time in the doctors’ care centers throughout Nassau & Suffolk as needed (most can be done via TEAMS or phone)

Qualifications

  • Bachelors in related field; Masters preferred
  • 3+ years experience working in similar programs (population health, utilization management, patient education, value-based models etc.)
  • Strong operational skills to develop workflows, policies & procedures, KPIs and reporting
  • Intellectual curiosity to learn new material, regulations and requirements
  • Superb communication skills, both oral and written
  • Ability to use data for organized and efficient decision-making
  • Ability to innovate to build a better model within the regulatory structures
