About the Role
As a Care Coordinator, you will play a pivotal role in shaping the operational excellence and strategic direction of Meridian. You will:
- Coordinate and conduct comprehensive, multi-domain assessment of client in conjunction with Team members.
- Provide coordinated care management services for clients
- Develop client service plans and coordinate care in conjunction with Team members.
- Conduct face-to-face visits with client to monitor progress.
- Initiate and maintain relationships with clients' family and community.
- Ensure that clients are enrolled in all mainstream benefit programs.
- Make referrals and facilitate enrollment in any patient assistance/medication assistance programs.
- Provide treatment, rehabilitation and support services.
- Implement shift management.
- Provide on-call duty as needed.
- Participate in providing substance use, co-occurring, physical health and other treatment services.
- Provide Electronic Medical Record documentation of client progress.
- Complete and maintain all required training.
- Research, develop and maintain a comprehensive list of community resources that includes email addresses, phone numbers, and names of contacts for each resource.
- Responsible for meeting yearly productivity and revenue goals.
- Performs other duties as required to meet program and departmental goals and objectives.
- Maintains a caseload of 20-40 clients depending on children/adult's mix.
What We’re Looking For
- BA/BS from an accredited university with major course work in the areas of health related/ psychology/social work areas with at 1-year experience in Care Management with severely mentally ill or similar populations
-----OR-----Certified Behavioral Health Case Manager (CBHCM) verified through the Florida Certification Board
-----OR-----bachelor's degree in counseling; Social Work; Psychology; Criminal Justice; Nursing; Rehabilitation; Special Education; Health Education; or a Human Services field in which major coursework includes the study of human behavior and development with 1 year full time equivalent working with persons with serious emotional disturbances
-----OR------Bachelor's Degree from an accredited University or College and 3 years' experience working with children and/or adults with severe mental health issues.
- Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements.
- ** Must be able to type at least 27 wpm on prescreen assessment and score at least 65% on general assessment.
