ABOUT THE ORGANIZATION
SaVida Health is a specialty medical practice providing outpatient opiate and alcohol addiction treatment services. SaVida Health's care model includes medical care, counseling, comprehensive toxicology testing, case management and medical management of psychiatric medications. SaVida provides compassionate and individualized treatment programs to support those with substance use disorder.
Substance Use Care Coordinator ensures that individuals with Substance Use Disorders (SUD) are connected to appropriate services, including behavioral health, medical care, and community supports. This role emphasizes patient engagement, care planning, service coordination, and collaboration with interdisciplinary teams to ensure comprehensive and compassionate care.
Role and Responsibilities
Substance Use Care Coordinator ensures that individuals with Substance Use Disorders (SUD) are connected to appropriate services, including behavioral health, medical care, and community supports. This role emphasizes patient engagement, care planning, service coordination, and collaboration with interdisciplinary teams to ensure comprehensive and compassionate care.
Role and Responsibilities
- Serve as the primary contact for assigned members and be available by phone or email.
- Engage members in their individualized plan of care.
- Communicate with members at least monthly (or as needed) to monitor needs and progress.
- Lead monthly interdisciplinary team (IDT) meetings to coordinate care across providers.
- Track and document member progress and care outcomes.
- Connect members with community resources, such as housing, food assistance, transportation, legal aid, employment support, childcare, and financial assistance.
- Coordinate with probation/parole officers and community corrections for justice-involved individuals.
- Assist members in obtaining public benefits and accessing services.
- Monitor members’ physical and mental health conditions and medications.
- Maintain timely, accurate documentation in Electronic Health Record (EHR) system.
- Maintain confidentiality.
- Other duties as assigned.
Qualifications and Education Requirements
Must meet one of the following:
Must meet one of the following:
Option 1:
Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling
And:
One year of direct experience or training in services for individuals diagnosed with SUD
Or:
One year of clinical experience or training with individuals with co-occurring SUD and mental illness
And:
One year of direct experience or training in services for individuals diagnosed with SUD
Or:
One year of clinical experience or training with individuals with co-occurring SUD and mental illness
Option 2:
Licensure as a Registered Nurse in Virginia
And:
One year of experience or training in services for individuals with SUD
Or:
One year of clinical experience with co-occurring SUD and mental illness
Option 3:
Certified Substance Abuse Counselor (CSAC), CSAC-Supervisee, or CSAC Assistant under supervision as defined by state regulations.
Preferred Skills
- Experience working with justice-involved individuals or coordinating care with correctional systems
- Strong knowledge of community-based resources and social service systems in the area
- Familiarity with Motivational Interviewing, Harm Reduction, and Trauma-Informed Care approaches
- Demonstrated ability to build trust and rapport with vulnerable or underserved populations
- Excellent written and verbal communication skills
- Strong organizational skills and attention to detail
- Proficiency in using electronic health records (EHR) and case management systems
- Ability to work independently and as part of an interdisciplinary team
