Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have an opportunity for a Care Coordinator in South Chicago, IL. We are looking for a dynamic individual to conduct determination of eligibility/need assessments, facility placement pre-screening assessments, and authorize Community Coordination Program (CCP) Services for all eligible Illinois Department on Aging’s CCP program participants.

Key Functions include:

• Administration of the appropriate intake form, including a comprehensive needs assessment.
• Development of a case plan.
• Making appropriate referrals and responding to applicant/client requests.
• Authorization of services.
• Maintaining case records, including documentation of follow-up, reassessment, and termination.

Qualifications:

• BA/BS degree in social science, social work Preferred or related field.
• Two years of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly.
