Behavioral Health Care Manager - Hybrid schedule, Hiring bonus!

Seeking a full time (37.5 hours/week) Behavioral Health Care Manager. The Behavioral Health Care Manager works within the Home Care Department under the Behavioral Health Outreach for the Aging Population (BHOAP) funding. The Behavioral Health Care Manager will be responsible assisting eligible older adult persons with recurrent, fluctuating behavioral health needs, to obtain and retain behavioral health services and Long Term Service and Supports (LTSS) services which will enhance their quality of life and allow them to live as independently as possible with dignity and respect. Excellent written, verbal, interpersonal communication and organizational skills required.

Qualifications: Bachelor’s degree preferred and behavioral health experience preferred. Desire and commitment to working with older individuals required. Excellent written, verbal, interpersonal communication, and organizational skills required. Computer proficiency, including experience with Microsoft products (Excel, SharePoint, Word) and working within a database. Ability to multi-task and establish priorities, in a fast-paced environment. Must have moderate physical mobility; Ability to lift and carry objects weighing up to 20 pounds. Use of a reliable vehicle including active vehicle insurance with a current license to drive required.

· A hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year of active employment. The bonus for this position is $1,000.

Non-negotiable salary of: $48,009.00 - $49,023.00 (bilingual) annually
