Why does the job exist?
Healthcare Surveyor/ Intake Coordinator will:
Be responsible for the intake of complaints for all licensed healthcare facilities and review of incident reports related to resident/patient abuse, neglect, exploitation, and injuries of unknown origin;
Investigate, evaluate, and triage complaints to include identifying imminent danger that would require immediate interventions.
How does it get done?
Healthcare Surveyor/ Intake Coordinator is responsible for:
Gathering incident reports and consumer complaints from all sources to include faxes, hotline calls and on-line system;
Acquiring the necessary and required information to adequately triage;
Triaging daily for possible assignments or facility follow up for internal investigation.
Who are the customers?
Residents/Patients receiving care in all Licensed Healthcare Facilities throughout New Mexico.
Ideal Candidate

Minimum Qualification
Associates Degree from an accredited college or university in Nursing, Architecture, Social Work, Sociology, Psychology, Rehabilitation, and/or Criminal Justice and two (2) years directly related experience in community based healthcare services and case management. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience.
Employment Requirements
Must possess and maintain a valid New Mexico Driver's License. Must Possess and maintain a current Defensive driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification as a condition of continued employment.
Working Conditions
Office setting. Sitting for long periods of time viewing computer monitor. Repetitive hand movement. Standing, walking, lifting up to 20 lbs. Use of computer, copier and telephone. On-call duties may be required on occasion. There may be some instances when onsite training is done at healthcare facilities.
