Job Overview
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will play a crucial role in providing support and resources to patients and their families, particularly those facing medical challenges. This position requires strong interpersonal skills and the ability to navigate complex healthcare systems while advocating for the needs of individuals with various health conditions.

Duties

• Conduct comprehensive assessments of patients' social, emotional, and environmental needs.
• Develop and implement individualized care plans in collaboration with healthcare providers.
• Provide counseling and support to patients dealing with chronic illnesses, disabilities, or mental health issues.
• Facilitate access to community resources, including financial assistance, housing support, and rehabilitation services.
• Document patient interactions and progress in accordance with medical documentation standards.
• Perform diagnostic evaluations to identify patient needs and recommend appropriate interventions.
• Utilize therapeutic techniques such as cognitive behavioral therapy and motivational interviewing to enhance patient outcomes.
• Work closely with families to provide education on dementia care and other age-related issues, particularly in geriatrics.
• Advocate for patients' rights within the healthcare system, ensuring they receive the necessary services and support.

Requirements

• Master’s degree in Social Work (MSW) from an accredited program.
• Valid state licensure as a Clinical Social Worker (LCSW) or equivalent credential.
• Experience working with individuals with disabilities or chronic illnesses is preferred.
• Proficiency in medical documentation practices, including familiarity with ICD-9 coding.
• Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
• Ability to work independently as well as part of a multidisciplinary team.
• Knowledge of community resources relevant to healthcare services is highly desirable.
