PACE Social Worker (MSW)
Pay Range: $46.15 – $52.70/hr
Requirements to apply:
• Master’s degree in Social Work from an accredited college or university
• Current BLS and First Aid certification
• Fluency in Chinese (Cantonese and/or Mandarin) required
• Valid California driver’s license with good driving history
• Preferred: Current CA LCSW, 2+ years of experience in a health-related setting, and 1+ year working with frail or older adults
Full-time day shift opportunity for a mission-driven Master’s level Social Worker to provide care management, counseling, and advocacy within a community-based PACE program serving frail and older adults.
Job Overview:
The PACE Social Worker serves as a key member of the interdisciplinary team, applying independent social work skills to support participants’ long-term care needs. This role includes conducting psychosocial assessments, developing care plans, coordinating resources, providing supportive counseling, and facilitating care transitions. The ideal candidate brings compassion, cultural competence, and experience working with older adults, while thriving in a collaborative care model that values advocacy, respect, and whole-person care.
