#PACE

Job Summary:

The PACE RN Care Coordinator coordinates and delivers medical care to Valir PACE participants. The RN works jointly with physicians, providers, other nurses, therapy, activities, dietary, social services and transportation as part of the Interdisciplinary Team to determine and monitor the care plan for each PACE participant in compliance with all national and state regulations/laws.

Education, Licenses, Certifications and Experience:

• Current Oklahoma RN License without restrictions
• Either one year working with the frail and elderly population or approved training.
• Current BLS certification
