Facility:
This Program of All-Inclusive Care for the Elderly (PACE) center provides comprehensive healthcare services to seniors. The center operates within a coordinated care model designed to help older adults maintain their independence while receiving necessary medical and social support services in a community-based setting.

Position Overview:
We are seeking a PACE MSW Supervisor to oversee and support social services within a PACE center. The Supervisor will provide leadership to social workers and case managers, ensuring high-quality, patient-centered care and effective coordination across interdisciplinary teams.

Job Responsibilities:

  • Oversee San Diego PACE Social Services provision under the Social Services Technical Manager
  • Lead social workers and case managers in delivering comprehensive social work services
  • May carry a caseload while training and mentoring staff
  • Provide weekly quality and productivity reports to Site Manager and Social Services Technical Manager
  • Ensure active participation of Social Workers in Interdisciplinary Team meetings
  • Train staff to conduct psychosocial assessments and develop care plans
  • Support hiring, onboarding, and ongoing professional development of Social Workers and case managers
  • Ensure timely completion of assessments, care plans, and Plan of Care documentation in EPIC
  • Escalate issues/concerns appropriately to the Social Services Technical Manager
  • Deliver superior customer service to participants, families, and stakeholders
  • Coordinate hospital discharge planning with interdisciplinary teams
  • Act as a resource for education on dementia, difficult behaviors, and other care needs
  • Ensure compliance with internal and external documentation requirements
  • May be assigned to special projects, including travel to other San Diego PACE sites

Qualifications:

  • Master’s Degree in Social Work from an accredited school
  • Valid California driver’s license and reliable transportation
  • Minimum 1 year of experience in a healthcare setting with frail/elderly populations
  • Knowledge of community resources and EPIC EMR documentation
  • Ability to conduct home visits and travel throughout San Diego County

Compensation:

  • Competitive hourly rates: $70 – $80/hour
  • Temporary Employed with benefits
