Evergreen Commons Rehabilitation & Nursing Center is looking for a full time Social Worker for our 41 bed short term rehabilitation unit.

Duties:

• Organizes and develops a plan of care based on their individualized needs of the residents. Asseses regularly to create goals and modify as necessary.
• Assists the residents, staff, and families with issues that impact or prevents adjustment or any other aspect of the residents quality of life.
• Assists with appropriate discharge planning including setting up services in advance to resident discharge.
• Consults with other members of the interdisciplinary team to provide adequate services that enhance the social and emotional well-being of our residents
• Consults with outside agencies if applicable. Responds to referrals and are initiated timely.
• Attends meetings as assigned
• Responsible for providing services to families where problems may interfere with resident's plan of care
• Complete documentation and assessments as required

Requirements

• Bachelors Degree and at least one year experience in long term care
• Organization and attention to detail
• Must be able to communicate and interact with all members of the health care team
