Overview
The Medical Social Worker plays a vital role in the hospice care team, providing emotional support, counselling, and resource condition to patients and their families. This position works collaboratively with physicians, nurses, chaplains, and caregivers to address psychosocial needs and ensure holistic care.
Key Responsibilities
- Assess the emotional, social, and financial needs of patients and families
- Provide counseling, crisis intervention, and grief support
- Assist families with care planning, advance directives, and community resources
- Advocate for patients' rights and ensure culturally sens
- Participate in case management activities to coordinate care across different service providers.
- Utilize EMR systems for data collection and record keeping related to patient care.
- Provide education on behavioral health issues, including PTSD care, dementia care, and developmental disabilities.
Experience
Candidates should have a robust background in social work with specific experience or knowledge in the following areas:
- Child & family counseling
- Hospital experience
- Hospice care
- Addiction counseling
- Behavioral therapy and applied behavior analysis
- Patient assessment and intake processes
- Clinical counseling techniques including motivational interviewing and psychotherapy
- Group therapy facilitation
- Crisis intervention strategies
- Experience working with diverse populations including geriatrics, pediatrics, individuals with disabilities, autism spectrum disorders, and Alzheimer’s care
The successful candidate will demonstrate proficiency in medical terminology, ICD coding (ICD-9 & ICD-10), CPT coding, as well as familiarity with clinical environments such as inpatient settings or nursing homes. A commitment to ongoing professional development through research and training is essential for this role.
Job Types: Full-time, Part-time
Pay: $80.00 per day
Expected hours: 20 – 25 per week
