You’ll be the trusted face of our home health and rehab services in Port Charlotte, FL, building relationships, uncovering new referral sources, and ensuring more families have access to high-quality, compassionate care. What You’ll Do

Grow referral relationships with hospitals, SNFs, assisted/independent living facilities, physicians
Educate providers and the community on in-home rehab and skilled care services
Use market data (Trella) to target high-potential partners and drive smart outreach
Coordinate closely with internal teams to ensure smooth patient onboarding
Track activity and results to stay organized and ahead of goals

What You Bring

2+ years of experience in home health, hospice, or healthcare business development
1+ years in B2B sales with connections in the area
A strong understanding of healthcare and referral networks
Drive, confidence, and the ability to connect with people at every level
Tech-comfortable and able to translate data into action
