Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and guidance to individuals and families facing various challenges, including mental health issues, developmental disabilities, and crisis situations. This role requires a strong commitment to patient care and an ability to work collaboratively with other healthcare professionals to ensure comprehensive support for clients.

Responsibilities

  • Conduct assessments to determine clients' needs and develop appropriate treatment plans.
  • Provide individual and group counseling sessions utilizing various therapeutic techniques, including cognitive behavioral therapy and psychodynamic therapy.
  • Engage in crisis intervention and management, ensuring the safety and well-being of clients during critical situations.
  • Collaborate with multidisciplinary teams to coordinate care and resources for clients, including case management services.
  • Maintain accurate medical documentation in compliance with HIPAA regulations, including intake forms, progress notes, and treatment plans.
  • Utilize EMR systems for effective data collection and management of patient records.
  • Participate in clinical research initiatives aimed at improving service delivery and patient outcomes.
  • Offer support in areas such as addiction counseling, PTSD care, child & family counseling, and behavioral therapy.
  • Provide home & community care services as needed, ensuring that clients receive the necessary support in their environments.
  • Stay informed about developments in social work practices and maintain professional development through ongoing education.

Skills

  • Proficiency in psychotherapy techniques, including motivational interviewing and crisis intervention strategies.
  • Experience working with children, individuals with developmental disabilities, autism spectrum disorders, and geriatrics.
  • Knowledge of medical terminology, CPT coding, ICD-9/ICD-10 coding systems, and medical records management.
  • Strong skills in patient assessment, diagnostic evaluation, and clinical counseling methodologies.
  • Familiarity with group therapy dynamics and individual counseling practices tailored to diverse populations.
  • Ability to conduct research related to behavioral health issues and contribute to clinical studies as needed.
  • Excellent communication skills for effective interaction with clients, families, and healthcare teams.
