General Summary:

This position works under the direction of the QRTP-Lead Therapist. Plans and coordinates after-care, discharge and placement of QRTP patients and their families/guardians under the direction of the Clinical Therapist. Works with the Clinical Therapists to provide case management services. This is a safety sensitive position when in direct contact with patients.

Duties & Responsibilities:

  • Provides case management services for assigned families.

  • Confers with parents/guardians and Clinical Therapists for an initial assessment of discharge needs of patients.

  • Coordinates discharge plans with multi-disciplinary team attending QRTP staffing and presents plans to DCFS, court, etc.

  • Serves as a liaison between Youth Home and the PASSE's.

  • Maintains regular contact with DCFS, assisting in finding appropriate placement for patients by participating in all transitional phone calls for patients.

  • Serves as a family advocate with other agencies that provide services, including but not limited to, juvenile court, schools, DCFS and others.

  • Maintains accurate documentation to ensure compliance with accreditation and auditing standards.

  • Schedules aftercare services and appointments for patients who are leaving treatment.

  • Assists patients in identifying and contacting appropriate community resources.

  • Other duties as assigned by supervisor.

Knowledge, Skills & Abilities:

  • Strong written and verbal skills; ability to read, analyze and interpret professional journals, technical procedures and government regulations

  • Ability to effectively present information and respond to questions from managers, patients, licensed associates and the general public

  • Ability to define problems, collect data, establish facts and draw valid conclusions

  • Basic understanding of calculation of figures

Education and/or Experience:

Bachelor's Degree required. Past experience in mental health and utilization of community resources is preferred.

Physical Demands and Work Environment

Team members are required to sit, stand, talk, see, walk and reach with hands and arms. Must be able to personally restrain patients. Must maintain a current tuberculosis skin test. While performing the duties of this job, the team member is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
