General Responsibilities:
Work Schedule: Sunday-Thursday
- Conduct Choices for Care assessments in hospitals, nursing facilities, supportive living facilities, and community-based settings.
- Inform individuals of all care options prior to admission and provide necessary resources.
- Determine eligibility for services from state, federal, and local programs.
- Provide active intervention and advocacy to access necessary services.
- Handle complex cases and work with clients facing multiple challenges.
- Monitor and follow up with clients as required by program guidelines through the completion of follow up phone calls and screenings.
- Complete emergency assessments and develop plans of care under Department on Aging guidelines for those older adult clients at risk for readmission.
- Submission of all assessments and supporting paperwork according to program timeline requirements.
- Create and maintain confidential case records and meet documentation standards.
- Maintain cooperative relationships with service providers and community organizations.
- Coordinate work schedule for adequate program coverage and utilize agency resources effectively.
- Serve as an agency ambassador in the community.
- Other duties as assigned within the guidelines of this position.

- Bachelor’s degree in social work, psychology, or related field from a regionally accredited institution.
- Minimum of one year of experience providing case management services.
- Experience with program-specific populations preferred.
- Strong communication skills, verbal and written, including interpersonal communication with older adults, family members, and interdisciplinary teams.
- Ability to work independently and respond appropriately in crises.
- Ability to relate well to individuals with disabilities and older individuals, respecting their privacy and dignity.
- Effective interaction with co-workers and community agencies.
- Competence in working with diverse cultures, family systems, and social backgrounds.
- Proficiency in Microsoft Office applications (Outlook, Word, Teams).
- Passage of complete background clearance and any other required testing.
- Valid driver’s license, reliable transportation, and proof of liability insurance.
