TriSource is seeking an experienced and compassionate Director of Social Work to lead the Social Services department of a long-term care and assisted living facility in Manassas, VA This role is responsible for identifying and addressing the psychosocial needs of the residents, ensuring regulatory compliance, and providing leadership to the social work team.

Key Responsibilities:

• Assess psychosocial needs and develop care plans to support resident independence and well-being.
• Supervise and support Social Work staff in daily operations.
• Oversee utilization reviews and assist with discharge planning and coordination with local agencies.
• Serve as a liaison and advocate for residents regarding Medicaid, SSI, Social Security, and disability services.
• Maintain accurate documentation and ensure compliance with federal, state, and facility regulations.
• Collaborate with interdisciplinary teams and external agencies for case management and crisis intervention.
• Lead conflict mediation and support abuse prevention efforts.
• Support admissions process and community relations initiatives.

Qualifications:

• Master’s degree in Social Work (MSW) from an accredited institution. LCSW preferred
• Minimum 1 year of experience in a nursing home, hospital, or social services agency.
• Experience working with the elderly or in long-term care preferred.
• Strong knowledge of Medicaid/Medicare, MDS, care planning, and resident rights.
• Excellent communication, organizational, and interpersonal skills.
• Clean background

Starting pay rate: $90,000/YR
