Bridge dialysis is opening new locations across Arizona to help support home hemodialysis programs in collaboration with skilled nursing facilities. Duties:

• Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the patient through the development of social service programs.
• Collaboration with medical professionals and other health related facilities and organizations to ensure proper support.
• Assist the QAPI Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
• Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
• Prepares for and participates in surveys (inspections)
• Participate in regularly scheduled reviews of resident care plans.
• Participate in patient discharge, development, and implementation of dialysis social care plans and patient assessments.
• Involve the resident/family in planning social service programs when possible.

Education and/or Experience:

• Must have, as a minimum, a master's degree in social work
• Must have one year of experience as a Social Worker, preferably in a hospital, long-term care facility, or related healthcare setting.
• Hold current license through the state board as a LMSW.
