Your Responsibilities:
- Conduct comprehensive patient intakes, establishing clear treatment goals and expectations while adhering to HIPAA and confidentiality regulations.
- Perform regular reassessments of patient needs, documenting progress and adapting treatment plans as necessary.
- Develop evidence-based, individualized treatment plans with measurable goals, collaborating closely with patients and their families or guardians.
- Provide psychotherapy sessions using tailored therapeutic techniques to foster positive behavioral and emotional changes.
- Collaborate with multidisciplinary teams to ensure seamless coordination of care across providers.
- Maintain thorough and timely clinical documentation in our electronic health records (EHR) system.
- Serve as a liaison between patients and external resources, ensuring appropriate referrals and follow-ups.
- Actively participate in professional development opportunities to enhance clinical skills and program effectiveness.
What You Bring:
Education: Master’s degree in Social Work or a related field with a clinical focus.
Licensure: Current licensure as an LCSW or LPC.
Experience: Minimum of 1 year providing psychotherapy and mental health services (internship experience considered). Minimum of 1 year working with Children and Adolescents
Candidates with a master’s or doctoral degree and active clinical licensure (LCSW, LPC) are considered.
Skills:
- Strong clinical expertise in assessment, treatment planning, and evidence-based interventions.
- Excellent communication and interpersonal abilities to build trust and collaborate with patients and healthcare professionals.
- Proficiency in using electronic health records (EHRs) and other healthcare software.
Knowledge: In-depth understanding of mental health conditions, treatment options, and the unique challenges faced by patients of all ages.
