Job Overview: We are seeking a compassionate and experienced Hospice Social Worker to provide psychosocial support to patients and their families/caregivers on our hospice services. The ideal candidate will assess the emotional, social, and environmental factors affecting the patient’s well-being, collaborate with the interdisciplinary team, and provide guidance through challenging end-of-life decisions.
Key Responsibilities:
- Psychosocial Assessment: Conduct assessments of patients' and families' emotional and social status related to terminal illness and their environment.
- Family and Caregiver Support: Offer counseling and crisis intervention as needed to support coping strategies and provide guidance during difficult times.
- Cultural Sensitivity: Assess and educate the interdisciplinary team on any special cultural needs or practices that may affect patient care, including communication, family roles, and traditions.
- Advanced Directives: Assist patients and families in understanding and preparing advanced directives, ensuring their wishes are communicated and respected.
- Resource Coordination: Provide information and referrals for practical and environmental needs, connecting families with community agencies and services.
- Collaboration: Participate in interdisciplinary team meetings, contributing insights on the psychosocial factors impacting patient care, and support the team in addressing emotional and social needs in care planning.
- Documentation: Maintain accurate clinical records for all patients referred for social work services and ensure compliance with organization protocols.
Qualifications:
- Master's degree in Social Work (MSW) from an accredited program.
- Current social work licensure in Ohio.
- Previous experience in hospice, palliative care, or a similar healthcare setting preferred.
- Strong understanding of end-of-life issues, advanced directives, and cultural competency.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Excellent communication, counseling, and crisis intervention skills.
