About Job

Overview
We are seeking a dedicated and detail-oriented Intake Coordinator to join our team. The ideal candidate will play a crucial role in the patient intake process, ensuring that all necessary information is collected accurately and efficiently. We are seeking a Masters and or Licensed Intake Coordinator to join our team of professionals. This role is essential in ensuring new clients are welcomed, clinically assessed, and provided with individualized treatment plans that align with best practices and state standards.

Responsibilities

Initial Outreach & Engagement

  • Contact new referrals in a timely manner.
  • Provide orientation to services, confidentiality, and agency processes.
  • Secure necessary consents.

Diagnostic Intake & Assessment

  • Conduct biopsychosocial assessments, clinical evaluations, and diagnostic impressions.
  • Administer standardized tools (e.g., PHQ-9, GAD-7, trauma screens).
  • Determine level of care and service recommendations.

Treatment Planning

  • Develop individualized treatment plans with measurable goals and interventions.
  • Ensure plans meet payer and regulatory requirements.

Documentation & Compliance

  • Complete all intake, assessment, and treatment planning documentation within established timelines.
  • Maintain audit-ready files compliant with AHCCCS, DCS, HIPAA, and agency standards.

Collaboration

  • Coordinate with case managers, therapists, and referral staff to ensure smooth onboarding.
  • Participate in case consultations and team meetings as needed.

Qualifications

  • Master’s degree in Counseling, Social Work, Marriage & Family Therapy, or related field.
  • Minimum 2 years of behavioral health clinical experience, preferably with AHCCCS/DCS populations.
  • Strong skills in clinical documentation and EHR systems.
  • Familiarity with medical terminology, behavioral health diagnosing, CPT coding, ICD-9, ICD-10 coding systems is essential.
  • Bilingual candidates are encouraged to apply for enhanced patient communication.
  • Strong computer skills
  • Excellent organizational skills with the ability to manage multiple tasks simultaneously.
  • Demonstrated customer service skills and a commitment to providing quality patient care.
