Intake Coordinator
Location: 337 E Virginia Ave, Bldg B, Phoenix AZ 85004
Hours: Sunday - Thursday, 11:00am - 8:00pm
POSITION SUMMARY:
Under the supervision of the Clinical Director, the Intake Coordinator provides comprehensive coordination for the referral, assessment, and admission of clients entering NAC programs. This includes eligibility verification, review of referrals and requesting of additional needed information, completion of assessments as needed, submission of prior authorization, and assists with admission into programming. Occasional weekend and or evening work will be required.

RESPONSIBILITIES:
  • Provides excellent customer service to clients and referring agencies
  • Receives and acknowledges incoming referrals (by fax, email, and phone calls/walk ins) while reviewing payor/insurance eligibility and creating client file in EHR within 24 business hours
  • Screens referrals for necessary clinical information and schedules clients for assessments upon receipt of referral.
  • Checks client eligibility regularly and submits for initial prior authorization for residential admissions when required and follows-up on all authorization requests
  • Notify clients and referral sources of all documents and needs for entering treatment
  • Assists with coordination of identified detox and/or MAT services as needed throughout referral, assessment and/or admission process
  • Completes assessments with appropriate recommendation for treatment, integrated service plans and related documentation.
  • Referrals made to internal programs as well as external providers as needed based on assessment
  • Prioritizes priority populations (i.e., Native American, pregnant, IV substance use, etc.) for residential admission on an ongoing basis to ensure high bed occupancy
  • Coordinates and completes residential admissions as needed and keeps the admissions center staff informed of client admissions schedules, status, and related issues
  • Maintains strong working and referral relationships with all existing contractors and collaborators, potential contractors and other numerous community and referral agencies
  • Attends meetings as needed to be fully aware of authorization and concurrent review processes
  • Support and actively engage in practices of inclusion through Cultural and Linguistically Appropriate Services (CLAS) Standards

EDUCATIONAL / WORK EXPERIENCE REQUIREMENT:
  • Bachelor’s degree in behavioral health field with 2 years’ experience in substance use or mental health treatment
  • OR Associates of Arts Degree in behavioral health field and 4 years’ experience (Behavioral Health Technician)
SKILLS REQUIREMENT:
  • MS Office proficiency
  • Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors.
  • Demonstrate excellent written and oral communication skills.
  • Excellent customer service skills.
  • Experience in working with the Native American population preferred.
  • Must demonstrate critical thinking, problem- solving and organizational and time management skills.
  • Proficiency in electronic health record systems
  • Must be able to work well with others in a team approach.
  • Possess and maintain a valid Arizona driver’s license
  • Fingerprint Certificates within 90 days of hire.
  • Bilingual- Spanish desired
  • Understand the dynamics of substance use and demonstrate the ability to integrate the knowledge for practical application.

  • Know the economic, educational, and social problems of Native Americans and referral sources available.
PHYSICAL DEMANDS:
  • Able to sit, stand walk with/ without accommodations.
NATIVE AMERICAN PREFERENCE:
Preference is given to qualified Native American Applicants in accordance with the Indian Preference Act. If claiming a preference, a copy of valid documentation will be necessary.

FAIR LABOR STANDARDS ACT:
This position is considered to be Non-Exempt and entitled to overtime pay for hours worked in excess of forty (40) hours per workweek for overtime pay provisions as provided by the Federal Fair Labor Standards Act (FLSA) and any applicable state laws.
