Duties
- Conduct assessments to identify clients' needs and develop individualized care plans
- Collaborate with healthcare professionals, community resources, and other stakeholders to ensure comprehensive care for clients
- Maintain accurate and confidential client records in compliance with regulations
- Stay informed about current social work practices and participate in professional development opportunities
Qualifications
- Bachelor's or Master's degree in Social Work or related field
- State licensed LPN
- Strong communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team environment
- Knowledge of social service resources and agencies
- Experience with case management and crisis intervention
Job Type: Full-time
Pay: $36,000.00 - $38,000.00 per year
