Job Description:

Assists patients and families with personal and environmental difficulties. Performs comprehensive evaluation for social, emotional, cultural, mental status, environments, and financial circumstances in conjunction with Interdisciplinary assessments. Recommends a plan of intervention based on patient's goals. Provide ongoing leadership to healthcare team to identify resources to meet patient's social needs post discharge. Provide ongoing education/support to the healthcare team

REQUIREMENTS:

Required to have MSW and IL LSW at hire.

Hospital experience preferred but not required.

Compensation:

Hourly Range - $28.12-$42.17

Incentives:

Eligible for evening/night, weekend shift incentive based on company policies and applicable job codes. Additional details will be provided during the interview process.

Bonus:

Potential for a performance-based bonus, subject to eligibility criteria and achievement of defined metrics. Details regarding bonus eligibility and performance goals will be provided during the interview process or upon hire.
