Job Title: Social Worker (LBSW) – Hospital Setting
Location: Brattleboro, VT
Duration: 13 weeks with possible extensions
Shift: Days | 8:00 AM – 5:00 PM | Monday–Friday | 40 hours/week guaranteed
Rate: $65 for LOCALS & $75-$78/hr for Traveler's
Job Summary
The Social Worker (LBSW) will provide clinical and case management services to patients and families across all age groups, promoting optimal individual, interpersonal, and social functioning. The role focuses on supporting patient well-being, assisting with reintegration into the community, and collaborating with interdisciplinary teams in a hospital environment.
Key Responsibilities
- Deliver clinical/case management services to patients and families in inpatient and outpatient settings.
- Assess psychosocial needs and develop individualized care plans.
- Provide counseling, crisis intervention, and supportive services for children, adolescents, adults, and older adults.
- Collaborate with medical and behavioral health teams to ensure comprehensive patient care.
- Support discharge planning and reintegration into the community or appropriate care environments.
- Maintain documentation and records in compliance with state and facility standards.
- Adhere to a flexible 40-hour work schedule based on patient and family needs.
Requirements
- Education: Master’s degree in Clinical Social Work (MSW) or counseling/behavioral health discipline (Required).
- Licensure:
- Registered on the Roster of Psychotherapists (Non-licensed & Non-certified) with the Vermont Office of Professional Regulation (Required).
- Vermont state license or eligibility (Required).
- Certifications: BLS (AHA) – must be willing to obtain.
- Experience:
- Experience working with children and adolescents (Required).
- 1–2 years inpatient social work experience (Preferred).
- Must be comfortable working with all age groups.
- Resume must list specific units and patient populations served (e.g., Behavioral Health, Medical Social Work, Long-Term Care).
Job Type: Contract
Pay: $65.00 - $75.00 per hour
