JOB SUMMARY


The Community Residence Social Work Case Coordinator is responsible for the comprehensive service coordination from admission to discharge. This includes planning the residents’ services, linking providers to children in the CR that need treatment, monitoring the resident’s treatment plan goals and objectives, and coordinating all phases of treatment. The Social Work Case Coordinator is the primary communicator with the families and all the providers working with the child in care.

The Social Work Case Coordinator documents in Precision Care all aspects of treatment. This includes individual crisis management plans, treatment plan goals and objectives, safety plans and ongoing progress notes. He/she plans and coordinates with the Residence Manager, home visits, family visits, sibling visits. They communicate between the youth in the program and family members. He/she further assists family members in understanding the treatment for their child and facilitates ways to assist family members in transferring these skills and interventions to other settings, especially within the home. The Social Work Case Coordinator also helps coordinate appropriate transition services for a youth nearing discharge from the program. They are expected to lead in information sharing with all additional support team members to ensure seamless care. These support individuals include Parent Advocates, SPOA team members and of course all house staff and administrators. It also includes all outside clinical and educational services for each individual child.

GENERAL RESPONSIBILITIES


  • Write treatment plan goals and objectives and individual crisis management plans.
  • Complete all CAIRS reports.
  • Ensure eligibility and subsequent payment from SSI, Medicaid and third party providers.
  • Facilitate home visits, family visits, sibling visits for the youth as well as communication between the youth in the program and family members.
  • Assist family members in understanding the treatment for their child and how to apply these skills and interventions to other settings.
  • Provide comprehensive service coordination from initial assessment to obtaining appropriate and adequate services for each resident.
  • Arrange appropriate transition services for a youth nearing discharge from the program.
  • Perform all additional necessary activities assigned to further the agency’s mission.
  • The Social Work Case Coordinator is responsible for entering into Precision Care all necessary documentation accurately and in a timely manner and in accordance with regulatory guidelines and agency policy.

  • Computer – The Social Work Case Coordinator must be able to use a computer and able to learn software programs.
  • Office Equipment- The Social Work Case Coordinator is required to operate a fax, copier, scan, and other office equipment.
  • Communication - The Social Work Case Coordinator needs to be able to speak, read and write fluently in the English language.
  • Writing - The Social Work Case Coordinator must be able to write in the English language using correct spelling, grammar, punctuation, etc.
  • Lifting - The Social Work Case Coordinator may frequently lift 0-25 lbs., occasionally lift 25-50 lbs., and rarely lift over 50 lbs.
  • Push/Pull - The Social Work Case Coordinator may frequently push/pull 0-25 lbs. of force, occasionally push/pull 26-45 lbs. force, and rarely push/pull over 45 lbs.
  • Sitting - The Social Work Case Coordinator may sit 50-60% of the working day.
  • Standing - The Social Work Case Coordinator may stand 10-20% of the working day.

  • Work environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee frequently works in an office environment. The noise level in the work environment is usually moderate due to talk and movement of the program participants and other co-workers. The noise level can increase on occasion due to yelling or screaming by the participants of the programs.

QUALIFICATIONS


  • You have at least a Master’s degree, preferably in Social Work with experience in Mental Health systems.
  • You preferably have a minimum of 2 years of experience in Mental Health Services.
  • You have professional maturity and demonstrate the ability to handle sensitive and confidential situations.
  • You have outstanding interpersonal and communication skills.
  • You are technology savvy with excellent command of a variety of computer software applications in word processing, spreadsheets, databases, and presentation software (MSWord, Excel, Access, PowerPoint)

HOURS AND TIME OF WORK


  • The Social Work Case Coordinator is contracted for 35 hours a week Monday through Friday from 9:00 AM to 5:00 PM. Some evenings, weekends and early mornings may be required.
