Support the mission of CAPP and become a key member of our interdisciplinary care team. In CHaD, Social Workers help parents to navigate medical systems, identify and reduce barriers to accessing care, provide grief/bereavement support when there are complex medical diagnoses or injuries, assist in connecting to community resources and are an integral part of the patient care team. In CAPP, Social Workers are present in all clinical visits. Social Workers conduct comprehensive psychosocial assessments, file reports with child protection, assist in safety planning with the family and child protection, coordinate and facilitate MDT meetings to include community agencies and provide brief supportive therapy to parents or children.
Responsibilities
- Provides complete psychosocial assessments of patients and families.
- Provides counseling to individuals and families to help solve social and emotional problems.
- Coordinates community resources to address/meet identified needs.
- Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
- Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
- Organizes support groups.
- Documents objective, concise and pertinent clinical data in the medical record.
- Perform other duties as assigned.
Qualifications
- Master’s Degree in Social Work (MSW) required.
- Social work experience in an acute, home care, or ambulatory setting preferred.
- Strong organizational skills, verbal and written communications skills.
- Sound decision making and judgment.
- Time management skills.
- Negotiation skills.
- Proven ability to deal with conflict resolution.
- Comfortable with basic computer skills.
Area of Interest: Allied Health- Pay Range: $56,430.40/Yr. - $87,464.00/Yr. (Based on 40 hours per week, otherwise pro rata)
