IMMEDIATE SUPERVISOR: Director of Behavioral Health
POSITION SUMMARY: A Social Care Services Care Manager helps individuals access resources and services that address their health-related social needs (HRSN), such as food/nutrition, housing, and transportation. The Care Manager is responsible for conducting initial eligibility assessments and screenings for individuals seeking services under the 1115 Waiver program. They connect individuals to appropriate community resources and support, develop personalized plans, and ensure clients receive the necessary assistance. Care Manager functions as a liaison between clients and service providers, advocating for their needs and ensuring they receive the best care possible.
QUALIFICATIONS:Minimum Education, Training & Experience:
Associate’s Degree in Healthcare or Human Services related field
Two (2) years of demonstrated experience working in healthcare
Or
High School Diploma or equivalent
Five (5) years working in healthcare
Functional knowledge of basic computer applications (Excel, word-processing, e-mail, etc.)
Preferred Education, Training & Experience:
Bachelor’s Degree in Social Work, Public Health, Human Services, or related
Experience providing care coordination, care management, or community-based client services
Or
Community Health Worker certification
Proficiency in Microsoft Office and Electronic Health Record (EHR) systems
Bilingual in English and Spanish
ESSENTIAL SKILLS AND ABILITIES:
Valid Driver’s License that meets agency standards
Ability and willingness to work a flexible schedule
Demonstrates strong customer service and interpersonal skills via phone, in-person, via email, etc
Ability to address multiple priorities, organize oneself and work without direct supervision
Ability and willingness to address questions and concerns with empathy and patience
Demonstrates familiarity with community resources and services
Ability and willingness to advocate for clients and their needs
Demonstrates culturally sensitivity and ability to work effectively with diverse populations
Ability to relate effectively to diverse groups of people from all social and economic segments of the community with a history of building authentic, constructive relationships with others.
Ability to comprehend medical insurance, billing, terminology and medical documentation
SALARY:
$22 per hour starting rate
